Thank you for scheduling a visit with Paris Orthopedics and Sports Medicine. To accelerate the patient check-in process, please download and fill out our new patient forms prior to your appointment. If you are unable to complete the forms prior to your visit, please plan to arrive 15 minutes early to fill out the paperwork in our office.
We offer parking onsite for the benefit of our patients. If you need assistance getting to and from your vehicle, please contact our office to make arrangements. We will gladly provide a wheelchair and/or arrange for assistance from staff members when needed.
Please arrive a few minutes prior to your appointment time to check-in. During this process we may need to update contact information, collect insurance information, fill out medical forms, and collect a copay. To ensure you are seen in a timely manner we suggest arriving 15 minutes prior to your appointment time.
Depending upon the reason for your visit, it may be necessary to undergo an x-ray prior to meeting with the doctor. Please be sure to dress appropriately, such as in shorts if you plan to have a lower body x-ray. Gowns or clothing will be provided as needed.
Once you are in the exam room, you will be evaluated by one of our expert providers. Please note that our providers will always give you their undivided attention during the examination process. Please be prepared to answer questions on your pain level, symptoms, and medical history. Your provider will complete a physical examination at this time.
Following your appointment, we ask that you checkout prior to exiting the building. At this time we will be glad to answer any administrative questions you may have as well as schedule your next appointment. If you need assistance getting to your vehicle, help will be provided.
Signing up with our patient portal allows you to access certain parts of your medical record, request an appointment, request prescription refills, and pay your bill.
